Job Purpose and Scope

  • Liaise with Support Coordinators to complete tasks.

  • Supporting our support coordinators to manage and coordinate participants’ NDIS plans to enable independence in the home and community. 

  • Being responsible for telephone phone and email inquiries, while providing excellent customer service to new and existing clients.

  • Support clients through the PML Support Coordination intake process.

  • Recordkeeping, operation, and management of the CRM System.

  • Maintaining a filing system.

  • Managing the scheduling of meetings and NDIS reporting requirements.

  • Assistance to the support coordinators as required.

 

Roles and Responsibilities 

  • Demonstrated knowledge and understanding of the National Disability Insurance Scheme.

  • Exceptional Customer service and problem-solving skills.

  • Reliability, flexibility, and commitment to high work standards.

  • Exceptional written communication skills and records management skills

  • A current unrestricted Driver's License including access to a reliable vehicle and comprehensive car insurance.

 

Knowledge and Skills 

  • High school diploma or equivalent 

  • Reliability, flexibility, and commitment to high work standards.

  • Exceptional written communication skills and records management skills

  • Proficiency in the use of Microsoft Office Suite and CRM systems

  • Strong written and verbal communication skills

  • Sound knowledge of the National Disability Insurance Scheme

  • Sound time management and organization skills

  • A current unrestricted Driver's License including access to a reliable vehicle and comprehensive car insurance

  • As per NSW Health Orders and DET requirements, all staff working within the disability sector must have at least one COVID-19 vaccination before 25/10/21.

Job purpose and Scope 

- Support participants to understand their NDIS plan
- Liaise with a range of stakeholders to secure suitable support for individuals living with a disability or illness, including requesting and arranging quotes from service providers.
- You will be responsible for managing and coordinating participants’ NDIS plans to enable independence in the home and community.
- You will be confident in the facilitation of effective working relationships and communications with the carers, family, external services, and other key stakeholders to help provide the most well-rounded support available for participants under NDIS support plans.

Roles and Responsibilities 

- Provide support to clients to prioritize and access an appropriate suite of available services
- Monitor and record support coordination as per agreed work schedule.
- Guide participants to navigate NDIS service providers and the supports that they provide to achieve their goals.
- Assist participants and family building capacity to navigate service systems.
- Record keeping and operation of CRM system.
- Promote workplace commitment to employment equity, diversity, WHS, risk management, and ethical practices.
- Collaborate with client/carer to complete Service Agreement, arrange bookings with the required services.
- Provide NDIS funded support coordination with careful attention to managing conflict of interest in line with Project Make Lemonade policies
- Help participants to optimize their plan, ensuring they get the most out of their funded supports.
- Link participants to the community, mainstream, and other government services.
- Support participants to elaborate on their goals and translate these into supports.

Knowledge and Skills 

- Tertiary qualifications in social science, case management, social work or Diploma level Community Services, Mental Health or Disability.
- A minimum of 6 months experience within a similar role
- Exceptional Customer service and problem-solving skills
- Reliability, flexibility, and commitment to high work standards.
- Exceptional written communication skills and records management skills
- Proficiency in the use of Microsoft Office Suite and CRM systems
- Strong written and verbal communication skills
- Sound knowledge of the National Disability Insurance Scheme
- Knowledge of regional disability industry service and support providers
- Negotiating the complexities of each unique situation, providing support in a caring and empathetic manner.
-  Ability to adapt to varying work situations and flexibility in resolving challenges
- Demonstrated ability to work autonomously as well as an integral member of a multi-disciplinary team
- Sound time management and organization skills
- A current unrestricted Driver's License including access to a reliable vehicle and comprehensive car insurance
